Church Resource Center
Plans and forms churches ask us for, free to take and use. No sign-up, no email wall. Each one is fillable; complete what you can and mark the rest N/A. If you want help finishing any of them, that help is free too: steve@crexchurch.com or 952-240-4547.
These documents do double duty. They make your church safer and better organized, and when carriers see them in a submission, they underwrite with more confidence. A church with written plans reads as a church that manages its risks.
Fillable Plan
Documents how your church prepares for, responds to, and recovers from an emergency, including a violent or threatening person. Safety team roles, communication, medical, fire, severe weather, and recovery, in one plan.
Carriers weigh a written response plan heavily. Keep a copy at the church and one off-site, and review it yearly.
Fillable Plan
The written policy for a volunteer armed security team: who authorizes it, who is on it, and the screening, permitting, and qualification standards behind it. Built for submission to insurance carriers.
If your church has an armed team, carriers need this documented. Law enforcement and professional firearms backgrounds on the roster are weighed favorably.
One-Page Guide
What to do at 150, 120, 90, 60, and 30 days before renewal. Carriers underwrite better when they have time; this is what time looks like, ready to pin to the church calendar next to your council dates.
A submission landing 120 to 150 days out gets read carefully and routed to the right desk. One landing at 30 gets a triage look. The calendar is the strategy.
One-Page Guide
The declarations page is the summary at the front of your policy, and most people file it unread. This walks it line by line in plain language: limits, deductibles, endorsements, and the one percentage line that surprises Midwest churches.
A steward who can read the dec page holds the whole insurance conversation. Silence on the summary is not coverage; this shows you where to look.
Fillable Form
The one form the whole carrier pool quotes from. Built backward from what every carrier in the pool asks, so one thorough pass prepares your church for all of them.
Also available as the online form, where every question carries a "Not Sure - Request Help" answer so nothing stalls you.
Fillable Form
Companion to the intake questionnaire for auto coverage. Lists every person who drives a church-owned vehicle, paid or volunteer, with the four pieces of information carriers use to run motor vehicle records.
A complete driver list is the basis of your auto rating. Missing drivers surface at claim time; listed drivers surface at quote time.
Competing carriers quote from real numbers, and the real numbers live in your current policy. The full policy is best; the declarations page, the "dec page" summary at the front of it, works when that's all you can find. Here's where it hides, in the order that usually finds it fastest.
Your current policy almost certainly arrived by email around last year's renewal, the same season you're in now. Search the inbox for your carrier's name, or "policy" or "declaration," around that date. When you find it, don't download anything: forward the email, attachments and all, to steve@crexchurch.com. One motion, done.
If someone saved the policy to a computer, look in Documents or Downloads, or search the machine for the carrier's name. Found it? Attach it right inside the online questionnaire, which carries upload slots for your policy files.
The packet your carrier mailed at renewal has the declarations at the front, usually a few pages marked "Page 1 of 4," "Page 2 of 4," and so on. Photograph or scan every page and upload or email them; confirm you have them all by the page numbering.
Under "Documents" or "Policy Details," download each policy as a PDF.
Your agent or the carrier's service line will send complete policies on request. This is a routine ask; it does not signal anything.
If the searches come up empty, Canopy Connect pulls your current details through a secure, read-only lookup. It makes no changes and notifies no one, and your login is never visible to Crex. Know its limit: Canopy often returns the declarations pages only, not the full policy, so when the full policy surfaces later, send it along.
However they surface, there are two ways in: attach them right inside the online questionnaire, which carries upload slots for your policy files, or email them to steve@crexchurch.com. Either way, we take it from there.
One questionnaire puts your church in front of the whole carrier pool. The plans above ride along in the submission and strengthen it. It's a private review; your current agent and carrier are not notified.
